Official Application Process for Community Membership at LymeCare Alliance

Introduction

At LymeCare Alliance, we value the commitment and contributions of our Community Members. Becoming a Community Member is not just about joining an organization; it’s about becoming a part of a global movement dedicated to fighting Lyme disease. The following guidelines outline the official process and rules for applying to become a Community Member.

Eligibility and Membership Application

Eligibility

Any individual who is passionate about supporting Lyme disease patients and advocating for better healthcare, with a genuine intent to actively participate in the missions, goals, and activities of LymeCare Alliance, is eligible to apply for Community Membership. Applicants must commit to engaging in meaningful activities that align with the organization's objectives and demonstrate their readiness to contribute to the community's efforts.

Membership Application

  • Declaration of Intent and Commitment: Applicants must submit a "Declaration of Intent and Commitment" as part of their application. This document should detail the applicant’s interest in LymeCare Alliance, their plans to contribute to the organization’s mission, and include the following declarations:
    • Acknowledgment of awareness and understanding of all organizational rules, missions, and goals.
    • Commitment to staying informed about updates and actively participating in the organization’s activities.
    • A declaration of legal eligibility to join the organization and a commitment not to harm the organization or society in any way.

How to Submit Your Application

  • Download the Required Documents
    Download both the Declaration Form and the Application Form.
  • Complete the Declaration Form
    Print the Declaration Form, fill it out, and sign it. Then, scan the signed document.
  • Complete the Application Form
    Fill out the Application Form electronically. There is no need to print, sign, or scan this form.
  • Submit Your Application
    Send the completed and signed Declaration Form, along with the electronically completed Application Form, to the official email address contacts@lymecare.org . Alternatively, you may submit the documents via another online platform approved by the organization.

Please ensure that both files—the signed Declaration and the completed Application—are attached to your email.

Review and Approval

Upon submission, the application will be reviewed by the Directors, Community Deputy, or Community Manager. Approved applicants will receive a confirmation of their membership via email or the online platform, along with instructions on the next steps. This confirmation will include an official document certifying their membership in LymeCare Alliance.

Application Document Deletion Policy

Upon acceptance or rejection of the membership application, your email, along with any attached documents or application materials submitted via other communication channels, will be promptly deleted.

Membership Responsibilities

Acting on Behalf of the Organization

Community Members may act officially in society on behalf of LymeCare Alliance Ltd. This includes engaging with external stakeholders, representing the organization in public forums, and advocating for Lyme disease awareness and support.

Reporting and Accountability

Community Members must submit bi-monthly contribution reports to the Directors, Community Deputy, or Community Manager. These reports must be sent to contacts@lymecare.org or submitted through the internal membership tools on the LymeCare.org platform. These reports will be publicly posted in the official achievements of the community, showcasing the ongoing contributions of members.

Ethical Conduct

Members must adhere to the highest ethical standards and avoid any conflicts of interest. Any potential conflicts must be immediately declared to the Directors. Members are also responsible for promoting the organization’s materials, documents, and missions, working in favor of the community and supporting the achievement of the organization’s objectives.

Termination and Removal

Voluntary Termination

Community Members may terminate their membership at any time by notifying the Directors and Community Deputy in writing via the general email address, contacts@lymecare.org .

Involuntary Removal

The Directors or Community Deputy may remove any Community Member at any time for actions or a lack of contribution that are detrimental to the organization’s mission.

Community Engagement and Transparency

Public Announcements

The organization may publish cases and decisions publicly, ensuring transparency and accountability in its operations. In specific cases, the Directors and/or the Ethical Review Board may decide to disclose the full information of cases to the public, maintaining the integrity and ethical standards of LymeCare Alliance.

Juror Participation

Community Members will have the right to give their votes and opinions on cases under ethical review, similar to the role of jurors. This participatory approach ensures that the community’s voice is heard and considered in all major decisions.
